You must complete the Informed Consent Form before you can proceed.

I, David Hawkins M.H. , an not a medical doctor nor a psychologist, and do not hold myself out as one. I am a Health Care Practitioner, a Master Herbalist, and a Consultant of Natural Health.

As my client, you will be taught a lifestyle of healthy living. This lifestyle is known as Natural Health, and is not substitute for medical treatment. Here, you will be educated in the effective, productive use of pure water, fresh air, sunshine, rest, exercise for relaxation, strength and flexibility, enzyme-active food, and the importance of attending a positive outlook. This is called natural living.

In response to the above declaration, I agree, to always seek medical advice for medical treatment, and that I am here to learn the effective use of Natural Health in my daily life.

I also acknowledge that nothing in the teachings of David Hawkins M.H., is for the diagnosing, treating, alleviating, mitigating, curing, or caring for "disease" in any way or manor.

I clearly understand that "diagnosis" or treatment of any kind for "disease", is outside the scope and practice of Natural Health.

I acknowledge for being here, is to learn Natural Health, and that all the teachings and methods administrated are for the sole purpose of assisting myself and others, in learning how to build our health.

I also authorize David Hawkins to release pertinent information from my files to my referring Health Care Practitioner.
